Den kliniska bilden vid respirationsformen av IBR är feber, matvägran, kraftig hyperemi i nosslemhinnan (red nose) som sedan blir nekrotisk, nos- och ögonflöde (först seröst, sedan purulent) och hos mjölkande kor nedgång i mjölkproduktion. Hosta kan förekomma.
