Inga specifika makroskopiska förändringar ses vid obduktion. Histologiskt ses en diffus encefalomyelit i framförallt grå substans i cerebrala cortex, talamus och hypotalamus, med nervcellsdegeneration, perivaskulära "cuffings", gliacellsproliferation och infiltration av polymorfonukleära leukocyter i den grå substansen. Inga inklusionskroppar ses. VEE kan även ge nekroser i pankreas, lever och hjärtmuskulatur.
